Objectives To delineate the clinical features and ocular motor findings in acute vestibular syndrome (AVS) associated with anti-GQ1b antibodies. Methods We reviewed 90 patients with positive serum anti-GQ1b antibody in association with various neurological syndromes at Seoul National University Bundang Hospital from 2004 to 2018.

Acute vestibular syndrome (AVS) is defined as the sudden onset of acute, ‘continuous’ vertigo (lasting longer than 24 hours), associated with nausea, vomiting and head motion intolerance, the term was initially introduced by Hotson and Baloh in 1998. Vestibular neuritis is an inner ear disorder that may cause a person to experience such symptoms as sudden, severe vertigo (spinning/swaying sensation), dizziness, balance problems, nausea and vomiting. It included 34 patients with vestibular neuritis, 18 patients with sudden sensorineural deafness with vertigo, 1 with vestibular schwannoma, 6 with acute vestibular syndrome with migraine, 3 with acute vestibular syndrome: affected patients present with sudden onset of intense and disabling vertigo. The vertigo is paroxysmal, aggravated by head movement, and associated with nausea and vomiting. The patient exhibits horizontal or torsional nystagmus or mixed horizontotorsional nystagmus. Vestibular neuritis is the most common cause for acute vestibular syndrome (acute vertigo with acute nystagmus).
